Memory Clinic

Memory Clinic--A team approach to the diagnosis and treatment of memory problems

The Memory Clinic provides the patient with ehlp for thier memory problems and offers educational resources to manage day-to-day living with as much independence as possible.

Is the Memory Clinic for you?

If you or a loved one has:

  • Declining function and decreased ability for independent living due to perceived memory problems.
  • Complaints of memory loss and family history of Alzheimer's disease.
  • Been diagnosed with a memory problem and are having difficulty remaining independent in the community or requiring increased family education and support services.
  • Been exhibiting subtle changes in behavior, personality, mood or work habits.
  • Misplaced important items, gotten lost, have been unable to locate a parked car or have made financial errors.

Talk to your personal physician and discuss the memory issues affecting you or your family. A multidisciplined and highly trained team of memory experts is available to help.

The Patient Evaluation

Each patient is seen by a team of memory professionals, consisting of a physician, a psychologist, and a medical social worker. Each patient is assessed for the following:

  • Safety, cognitive and perceptual skills.
  • Depression and anxiety levels as well as social support systems.
  • Impairments dealing with hearing, sight and other medical conditions.
  • Communication and cognitive skills.

This half-day evaluation results in a set of recommendations tailored to the patient's specific issues, needs and abilities. These recommendations are provided to the patient's personal physician and will be reviewed with the patient and their family in a follow-up visit with the memory team. Recommendations may include referrals for further evaluations or treatments.

The Memory Clinic is held every Friday.  New patient evaluations begin at 8:30 am and are completed by 12:00 noon.  Medicare, Medicaid, and some HMO plans cover these services.
